Normally, I look up the car price on Hotwire and then reduce it about 20-25% and bid on Priceline, eventually beating the Hotwire offer. This time, Hotwire was so cheap, I didn't even bother. Both Hotwire and Priceline use Hertz, Avis, Enterprise, Alamo, and Budget, as I have had cars from all of those.
This approach almost always works to get a much less expensive car. For example, on our most recent trip, the airport rental for a week with a full sized car was over $600. The location less than 2 miles away was $380 on the website. Calling directly to the location (not the 800 number) got it down to $290.
I booked a "Hot rate" car rental through Hotwire for $402. I got the confirmation email and checked the site of the car rental company. Their price is $266 -- same location, car, dates, and times. Hotwire has a "low price guarantee" prominently displayed on their site, so I made a claim with Hotwire with screenshots, links, etc.

So a better deal on a base price will result in the add on fees being a higher percentage of the total. I looked at a receipt I have for a car rental in San Francisco. With discounts I got a great base price of $12 per day. My 3 day rental base price 12 X 3 = 36. They added $29 in fees, so the fees were almost as much as the base price.
