    Flying to the US in early Feb with BA. There is an attestation form to complete to enter the US. I have heard nothing from BA re this form and can find little on their site regarding it. Can anyone advise how you complete it and evidence this for the airline? Do I need to print it out as a hard copy to take to the airport?
