You should dispute the charges through your credit card company and also post your complaint of their FB page (Avis, not Costco). Their PR people read them all the time and respond promptly.

   Dear AVIS Car Rental, Shame on you! After reading all the complaints and warnings of Costa Rica Car Rental companies and their questionable extra charges, we settled on AVIS. We reserved through the AVIS head office, and got our written estimate in hand. We were aware of the Mandatory insurance as well.

   At booking, you have the choice of paying the full or 50% of the cost of the rental. They will simply ask you to pay for the rest before they hand over the keys. This company is definitely my new go-to car rental when I go to Sao Miguel. They are the most affordable and have the most automatic options.
