As Nick has stated, read the terms for the credit card you plan to use. Most cards insist you decline all other insurances but in Italy the price of the rental includes some insurance so you can't decline it, which is why most cards don't cover Italian rentals. Again, read the terms and conditions of the credit card to be certain.

More bad news....if you don't own a car, and therefore don't have your own auto insurance, you'll have to purchase liability insurance to go with your car rental.Might be best to go through something like AutoEurope.com, and choose a rate that includes liability coverage (your credit card probably gives you automatic collision coverage, but not liability).

Basic Liability protection is no longer included in the rental rates in California. E.g. Avis's policy: "With respect to rentals commencing in California, the Rental Agreement does not afford you, or any other driver, any insurance or protection against liability in those two states." I usually buy my own CDW + Excess protection + SLI.
