If insurance was included in your booking, then it should have been reflected on the booking - you should have had something to show that insurance was included.

Yes they do. They are part of the car rental shuttle system that services, hertz, budget, Avis, National and Advantage car rental companies.Depending on which airline and or terminal you fly into, it can be a quick walk like mentioned above or if you use hawaiian you will need the shuttle as the the walk would be long and annoying with luggage.
Some Hertz locations are just that - painful to deal with. Others are nicer. Hertz 800 number is 1 (800) 654-4173 so you might want to see if they can direct you to someone who is not at the location you rented from. (I would not give it much hope but I would suggest trying to contact people outside of the location you rented from if possible).
