It SEEMS to say in the fine print that from the time you make the first call to the U.S., as long as you stop using the plan before 24 hours are up, you can go as many days as you want without using it and without paying for it, and then when you do use it, you've got another 24 hours of use.

It costs about $1.25/minute, but reception is good. You will not have a problem in St. Martin with T Mobile.

For my phone, this meant obtaining a unique code for my phone from ATT, and me entering the code sequence into my phone. Getting the code wasn't easy, because mainly ATT seemed to pretend that they didn't know what I was asking about. I think I spent about 45 minutes on the phone getting the unlock code.
