The UAE is assigned an international dialing code of +971 by ITU. Telephone numbers are fixed at seven digits, with area codes fixed at two or three digits.

So if you use your US sim card you will always have to dial +49 to call a German number. +49 replaces the "0" that you would dial when calling a German number from a German phone. You do not have to dial the area code when calling a land-line number from a land-line which has the same area code.
