Most other airlines do not. Usually, it's a $150 change fee per ticket plus any refare, for a domestic ticket.

Re: United Airlines reimbursement after flight cancellation. Sep 23, 2023, 12:18 PM. If the cancellation was for reasons beyond UA’s control ( ATC, weather, etc.), you will not be reimbursed. And, if you are eligible for reimbursement. You are limited to $200, which includes hotel, meal and transportation.
Refund on United Airlines for medical reasons. 12 years ago. My son broke his leg a few days before a family trip and we needed to cancel our flight on United Airlines. I called 3x and got 3 different answers (BTW, each call took over an hour): they give you back cash minus $50 service fee. they give you credit minus $50 service fee.
United does not send a form. You need to get your doctor to write a letter on official letterhead and sign it. Then do the following. In order to get a medical waiver or refund, you go to the Refunds page on united.com and follow the following links.

United bumped my daughter from a Thanksgiving week flight due to overbooking and promised to mail a refund check to me (the purchaser). The first week of December they told her it was in the mail. One month later, it's still not here. I went online to the refund page but when I put in her ticket number it came up "not found."
8. Re: United refund policy on medical condition. 10 years ago. I suspect the agents who process them have a lot of discretion-I had a very positive experience-I had to cancel my trip CID-BOM-CCU last November- I had an abdominal surgery for cancer-they refunded everything, including the E+ fees.
