Hello, I just arrived to Iran 12 days ago and I applied for the 15 days VOA. I got it, in the form they gave me at Khomeini airport at Tehran they asked me to put a host (I'm not sure if it's strictly necessary to fill those spaces). I know a family in Iran so I put their info (name, phone number, and don't remember if also address).
