You'd need to check the Customs form to be 100% sure but I've a hunch that you only have to declare medications if you're bringing in commercial quantities. Prescription meds for personal use usually are treated differently.
If you check with your pharmacy it's possible they could print a set of labels, though I wouldn't be surprised if that turns out to be a federal crime.
