What I understand is that Tokyo has five different rail systems, which are, the monorail, JR, other private rails, metro, and subway besides local buses. Each has its own ticket system and separate entryway even if they are located at one station.

3. You need to go to the rail operator's website (JR if going between Tokyo and Kyoto). You can buy in advance though in mid-December, you can also easily buy on the spot.
