I've entered the Philippines through Clark, Manila, Iloilo and Cebu in the past few months. The Customs Declaration form is no longer given out on the aircraft but is available either as you step into the terminal or just before immigration. The form is no longer needed if you have nothing to declare in MNL CRK and CEB.
