For rental cars, they work off license plates. In Texas, Hertz registers their cars with PlatePass, their third party partner for tolls. When the car is so registered, PlatePass gets the bill and processes it for Hertz, charging your card a fee for each day you use a toll road, plus the usual pay-by-mail toll.
The bill needed paying so I went on line and keyed in the Plate Pass web site. Entered my Hertz rental agreement number and proceeded to pay the $19.50 by my Visa card, here comes the "con", right at the end of the process the dollar sign changed to a pound sign - I was about to be charged £19.50 not $19.50.

Hertz charges $5.95 for each day that a toll is incurred, plus the undiscounted toll itself. They use PlatePass to process toll charges, match the license plate to your rental period, etc., and it won't appear on your rental bill but will appear a few weeks later on your credit card. You don't have to do anything.
