In the US , insurances cover the driver, regardless of the car he is driving. Additional insurance covers excess on the drivers policy. So normally, an accident is covered by the driver’s insurance policy. Now, most credit cards provide additional insurance policy when used to make a car rental reservation. Check with your bank.

I have also purchased it along with my travel insurance plan, several companies do that and it's cheaper than buying from the car company. I don't think US car rental insurance companies typically include coverage of foreign rentals in the regular insurance. Mine doesn't, anyway, which is why I buy it.
