I feel your "pain" but it may not work to continue to contact the place you ended up renting from. Some Hertz locations are just that - painful to deal with. Others are nicer. Hertz 800 number is 1 (800) 654-4173 so you might want to see if they can direct you to someone who is not at the location you rented from.
We rented a car from Hertz on 5/24/13 to 6/14/13. We made arrangements for the rental through AAA in the United States. We picked up the car at London Heathrow. We paid 484 pounds sterling for collision damage waiver (equivalent of US $735).
