On April 25th Ministry of Health for Tanzania announced the launch of the online form for the traveler's health declaration. This must be completed within 24 hours before arrival in to Tanzania and once completed will issue a unique health code which should be presented upon arrival. Re: Tanzania online visa process, payment problems and possible. @Western_Traveler, agreed about Kenya — they have their act together with the evisa (and joint visa with Rwanda and Uganda), to the point where they’ve been able to eliminate the visa on arrival. Tanzania has a long way to go to get to that point.
Re: Health Form. 2 years ago. The health form was part of the requirement to enter Tanzania up until May when the link to the form was taken down for an update. There currently isn't a health form required / available. I recommend you keep checking the health form link before you travel just in case it goes back up https://afyamsafiri.moh.go.tz/.

I'm afraid it's a scam. They took USD 119 per person from our creditcard (this amount wasn't mentioned beforehand) and now we can't get in contact with them. We had to get our creditcard blocked. I've called the Tanzanian embassy and they said to just request the visa on arrival. Report inappropriate content.
