The general thoughts were: no need to buy the ins. from the rental company because your own car insurance company, if you have full coverage, will be sufficient. There was also talk about the fact that you most likely will need a credit card at the rental desk, and this would/could be another way of covering you if you have an accident.
As Nick has stated, read the terms for the credit card you plan to use. Most cards insist you decline all other insurances but in Italy the price of the rental includes some insurance so you can't decline it, which is why most cards don't cover Italian rentals. Again, read the terms and conditions of the credit card to be certain.

We charge the full rental cost to our AMEX card and opt for their premium car rental insurance, which makes them the primary insurance company, NOT the secondary insurer as some credit card programs are. Cost is $16.95 TOTAL for up to a 30 day rental.
