Usually used at China Construction Bank and Bank of China branches. I've also at times used my BofA ATM card with 4 digit pin with no issues, mostly at China Construction Bank. The one time in Asia I ran into an ATM that wanted 6 digits was in Siem Reap, Cambodia last year and found that if I added two zero to the pin the ATM took it just fine.

Re answer 1, no Italian Bancomats impose a fee for use. Any fee will come from your own bank. It is quite possible, of course, that your own bank will not charge you a fee if you use a specific bank's Bancomat. And we use a 4 digit PIN for our ATM/Debit Cards and have never had a problem.
